  • Collaborates with other departments, such as Referral Coordination Initiative, Community Care, Intense Case Management, other VA hospitals, Nellis Air Force Base, etc., to ensure the Veterans' needs are met.
  • Guides patients through the healthcare system by assisting with access issues and serves as a resource nurse for the Cardiology Clinic. Some examples are obtaining a Beneficiary Travel Consult from the PCP for transport needs and requesting booking on behalf of the Veteran. The coordinator works with the PACT team to assist the Veteran in scheduling lab tests in other VA locations outside of the city, like Laughlin.
  • Tracks the progression of the community referral to ensure the veteran is scheduled with the proper specialist, receives the correct test/procedure, no duplication of tests, and returns to the LV VA to follow up with their primary cardiologist. The goal is to achieve patient satisfaction and cost avoidance.
  • Requests for patient records in the community, especially in relation to the community care referral.
