• Created 6 innovative social media strategies, strategically addressing various demographics (both in English and in Spanish) using Canva tools to increase client referrals for testin and linkage
• Collaborated with 4 formal community partners to conduct wide-scale testing events as part of the EHE grant and Community Health Initiatives grant

• Provide solutions through linkage and coordination of services to over 60 clients residing across 15 counties in Area 3/13 (North Central Florida) in partnership with the Florida Department of Health
• Referred clients to local resources to address barriers in medication adherence such as food insecurity, public assistance programs, dental services with contracted providers and transportation assistance
• Developed and maintained community resources per each of the 15 counties and relayed as well as referred clients to resources as appropriate
• Results: Enrolled various clients within 6 months during clinic flow and collaborated with schedulers and ID physicians in Ryan White enrollments

• Facilitated support plan meetings and quarterly review meetings in accordance with the i Budget Waiver Handbook through the Agency for Persons with Disabilities
• Researched and presented multiple bids for approval of funding through the iBudget waiver
• Coordinated and establish 88 contacts with a caseload of 45 individuals qualified under the Medicaid Waiver program residing in Alachua County, in which all contacts included face-to-face encounters in addition to telephonic encounters
• Assisted and coordinated linking services that are of need among clients within the community including but not limited to Vocational Rehabilitation Services, Life Transition Skills, Adult Day Training workshops and other community supports

• Facilitated transitional, exit and discharge conferences with local services including but not limited to Juvenile Probationary Officers, DCF, parents/guardians, aftercare personnel (Project Connect and Project Bridge), as well as community enrichment resources within Circuit 8, District 1
• Collaborated with case management, mental health, medical, recreational and educational departments to provide achievable transition goals for youth to attain upon completing performance plans as part of program compliance
• Developed community resource guide for various circuits
• Completed and complied to DJJ standards during 2020 audit
