As a Care Coordinator I assist patients in removing barriers to care. A lot of my patients are not local to Portland, so I help them find labs or imaging centers close to home. I have established wonderful communication with my team to provide quality care to our patients. Being able to engage and build strong relationships with outside organizations helps our patients receive their care in a timely manner. Im currently working with my leadership to develop best practices for several different teams in the department to support excellent patient care and reduce Patient Safety Incidents. I am also on a team with my supervisor to create a new time off consensus within our group to align with the union contract and business needs.

  • Working at CHO helped me gain confidence in teaching and being a resource to my co-workers. I was able to help patients effectively in person and over the phone. I was always up for learning a new skill from referrals to improving scheduling technology within the world of infusions. I assisted in streamlining workflows to other clinics within the department. I was a part of the remodel task force and gave advice to management to optimize patient care and staff only areas. I was always available to help out other clinic sites in need and could perform my duties successfully while building positive relationships.

  • At the SBHC, I was the go to person, I made sure students, staff and families were comfortable and confident using the center. I demonstrated leadership skills in this role by implementing projects and work flows and getting by in from other staff in the center. I was constantly managing several different job tasks while also identifying issues and working to find creative solutions. This role helped me become proficient in adapting to new and changing objectives in healthcare. I gained knowledge about getting and maintaining certifications with the state and certain programs. VFC, Board of Pharmacy, HRSA, OHA. I worked independently while also coordinating care with the other entities within the SBHC to make sure the needs of the students were met. I have proven to work well in a fast paced environments while also paying attention to detail. It was an honor to provide great costumer service to the students and successfully provide inspiration and motivation to my peers.
