- Since 2002, I have been employed as a Medicaid Waiver Case Manager. During this time, I have always held a case load of 30-70 cases all over Southern Indiana. Currently I am certified for the Health and Wellness waiver and Traumatic Brain Injury waiver.
- I have worked for the following Waiver companies; Factors, IPMG, Columbus Organization and CareStar. I am currently employed by Champion Support Services. I also worked for eight months at Alternative Care Solutions setting up their Structured Family Care program.
- I have a great working relationship with providers; understand the requirements of the state for the programs I work with and have always had over the twenty hours required of yearly training. I have assisted my clients with Medicaid issues and have provided training to new case managers.
- Assessed client needs to develop individualized service plans for Medicaid waiver eligibility.
- Coordinated with healthcare providers to ensure comprehensive support for clients.
- Monitored client progress and adjusted care plans based on evolving needs.
- Facilitated communication between clients, families, and service providers to optimize care delivery.
