Perform social work assessments and interventions for patients and families in the ICU, NICU, medical, surgical, and telemetry floors. Work collaboratively with a multidisciplinary team. Complete mental health and suicide assessments, provide brief counseling, crisis intervention. Assist in complex discharge planning. Provide supervision for Master of Social Work interns. Case Manager for 20 chronically homeless individuals with the primary goal of housing. Assist with community resources such as health insurance, nutrition resources, housing applications. Provide support to newly housed clients with life skills training. Team Lead beginning May 2014.
