• CNA/Caregiver (facility&homecare) General Purpose

• Observes and monitors the client’s condition, notifying Case Management staff of any significant change in client’s status and monitoring safety
• Provides housekeeping, home management, and laundry to maintain a safe environment
• Assists and/or provides all aspects of personal care including bathing, dressing, oral hygiene, hair care, and shaving
• Assists clients with ambulation and transfers as needed
• Helps with specialized assistive devices like Hoyer lift and the like
• Assists with prescribed exercises and rehabilitation
• Assists with eating and toileting as needed
• Prepares meals, planning, and shopping for regular and specialized diets
• Observes and reports intake of fluids and food and frequency of elimination if indicated
• Assists with medications that are self-administered

• General Purpose
• Provides basic patient care according to the skills and competency guidelines set by the N.C
• Division of Facility Services and the North Carolina Board of Nursing, Nurse Aide Central Registry
• Provide acute and intensive patient care under the supervision of the Registered Nurse
• Performs direct patient care based on the requests and needs as specified by patient and family, being age appropriate
• Provides assistance with activities of daily living: bathing, shaving, mouth care, skin care and progressive mobility
• Obtains accurate vital signs and document timely
• Report findings outside normal parameters to nurse for appropriate intervention
• Assists with patient nutrition to include feeding and proving supplemental snacks
• Performs intentional hourly rounding and anticipates patient needs
• Provide care to acute care and intensive care unit patients as needed.

• General Purpose
• Providing patient care to psychiatric and substance abuse patients as directed by a Registered Nurse and according to the plan established by the treatment team
• Participates in department and treatment team programs via educational groups and 1:1 interaction
• Provides patient care as delegated by the Registered Nurse
• Collaborates with Registered Nurse to plan and initiate appropriate interventions consistent with policies and procedures
• Observes and reports symptoms, behaviors, and changes in patient’s condition in a timely manner
• Initiates one-to-one interactions with patients according to the milieu program
• Maintains patient rights in accordance with North Carolina’s Patient Bill of Rights
• Serves as an adult role model for all patients and peers
• Formulates recommendations to improve individual performance.
