Coordinate nursing care through staff assignments, assisting and rounding with physicians, monitoring patients orders and communicating with other members of the IDT. Supervise and manage daily scheduling and tasks. Assist in training nurses in proper technique, care standards, operational procedures and safety protocols. Monitor nursing care, patient assessment process and treatment implementations. Document information on patients electronic medical record, relaying information to clinicians and care providers to facilitate continuity of care. Generate monthly reports to assure patients are meeting clearance and that patients access' are functioning without any issues. Continue to do all tasks as performed as team leader. Assist nurses, PCT's and other members of the IDT as needed.

Currently CAP 2 RN. Roles include direct patient care. Assessing and medicating patients according to orders provided by their nephrologist. Entering new orders as ordered by physician. Cannulating and connecting Central Venous Catheter patient so to receive treatment. Carrying out referrals made by physician for patients who need additional care, for example cardiology, podiatry, home health and wound care referrals. Medication reconciliation. Calling in medications, new orders and refills to patients home pharmacy and Fresenius Rx. Overseeing patient care technicians and assisting them as needed. Rounding with doctor if needed when Charge nurse is not on site or available. Team leading, assisting staff including social worker, dietician and front office with patients need. Team player, and do my best to remain accessible to both patients and staff.

To provide comprehensive benefit design support to external clients. Ensure accurate benefit maintenance of existing client information and implementation of new client information into the corporate system. Ensure appropriate service levels are maintained to meet corporate standards for performance and quality. Working under general supervision with some degree of independence and creativity. Primary functions are to receive assigned tasks, interpret client requests, and e-mail confirmation of interpretation to client for sign-off. Write code design requirements. Follow and document accuracy, timelines, and communication with the client from start of project until the end. Provide technical pharmacy benefit support to Account services, Clinical, Corporate communications, Customer service, and Information technology as needed. Maintain First Data Bank drug updates for current internal and external clients and program their requests based on responses received.
