AOT/Conservatorship Coordination - extensive outreach to a diverse population of individuals suffering from severe mental illness and substance use disorders. Assess appropriateness of referrals and eligibility requirements for the AOT program, locate and engage referred Individuals with local mental health resources, encourage voluntary participation in treatment and engagement among referred Individuals, provide crisis intervention and petition the court to order individuals into outpatient treatment when indicated, and provide peer and family support. Act as liaison between clients and community service providers, escort/transport clients to appointments, errands, and court appearances, provide courier duties for client medications and supplies. Maintain records and comprehensive case documentation using EPIC, Avatar, and department specific database.

Carry a comprehensive social services caseload of 300+, which includes case assessment and the development of service plans, performs a variety of specialized social work services. Conduct intake and reassessment home visits with aged, blind, and disabled individuals to assess the need for IHSS, verify program eligibility and authorized services, determine extent of supportive services needed, and develop treatment plan as well as referral and follow up services. Provide informal counseling and social support to clients and families. Interpret County and State rules and regulations to applicants and recipients. Respond to telephone and email correspondence from recipients, authorized representatives, and other departments. Update cases accordingly and take necessary action when notified of changes in client circumstance. Complete accurate and timely case documentation and reports. Prioritize work on daily basis to meet division regulations and deadlines. Establish and maintain records through CMIPS and other client related programs. Utilize MEDS, Compass Pilot, and CalWIN daily to access client information. Collaborated with multidisciplinary teams to ensure appropriate care coordination and resource allocation for clients. Educated families on available community resources, connecting them with essential services such as housing assistance, financial aid, or healthcare providers.

Determines initial and ongoing eligibility for individuals and families who apply for and/or receive public assistance through telephone and in-person screening. Conduct in-person and phone intake interviews with clients in regard to CalWORKs, Medi-Cal, and CalFresh applications and eligibility. Process emergency homeless assistance and vouchers. Review case documentation, verifications, and narratives. Process CalWorks, Medi-cal, and Calfresh applications, documents, and necessary verifications required to complete intake process and Inter-County Transfers under expedited time constraints. Prepare detailed case narratives daily.

Process Expedited Medi-Cal Applications (EMCAP) for county inmates and terminally ill clients, process all CDCR and AB720 applications in entirety. Actively participated on Contra Costa County Medi-Cal Committee. Utilize CalWIN, MEDS, Compass Pilot, Covered CA, etc. daily to verify/access client information.

Coordinate and implement youth services, i.e. workshops and employer events, at the One Stop Career Center. Instructed workshops for Resume Building, Interviewing Skills, Stress and Coping, Conflict Management, and Financial Planning. Complete responsibility for case management of 30+ at risk youth, ages 16-24 under the Workforce Investment Bureau (WIB). One on one weekly counseling with clients concerning barriers and programs available to overcome those barriers which included mental illness, drug and alcohol abuse and addiction, and probation/parolees obstacles, etc. Responsible for coordinating summer internships and placing clients in CTE programs with federal grant funding. Served as liaison between clients and employers/instructors. Conducted weekly site visits for all youth on my caseload. Conducted regular evaluations of program effectiveness, making adjustments as needed to ensure continued success in client transitions. Coordinated financial assistance programs for eligible clients seeking further education or certification courses related to their desired industry. Assisted clients in developing realistic expectations about the labor market and potential career options, promoting informed decision-making processes. Designed and delivered training programs on interview skills and networking strategies, boosting client confidence and marketability.
