Continuous Progress Form

  • Under the professor's guidance, the student works independently toward completion of the course whose description is listed in the catalog. A $100.00 fee per course over and above the regular tuition is charged.
  • In order to register for a CP course, this Continuous Progress form must be completed and signed by the professor and student which states the definite plan for the accomplishment of the course objectives. The plan must include a course syllabus and a work schedule defining blocks of materials to be completed with specific appointments and deadlines required. Necessary conferences must be worked out between the student and the instructor. The professor and the student must agree upon three specific subsequent times to review course progress and answer student questions during the semester of study. According to the expectations of the Southern Association of Colleges and Schools (SACS), opportunity for professor-student communication must be provided as agreed upon in the written Continuous Progress Course Schedule Form.
  • The Dean of the College must approve the Continuous Progress request and sign the Continuous Progress form before the student registers for the course in the Registrar's Office. The original form is taken to the Registrar's Office when the student registers. A copy is provided to the student, the professor, and the Dean of the College.
  • If any additional space is needed on the form below, please attach another sheet rather than writing on the back. The second page with the signatures is the only page that must be turned in. The registrar's office will attach the first page for imaging and documentation purposes.
