  • Participate in weekly Case Manager conference calls to corporate officials to insure continuity of care with regards to discharge planning
  • Submit weekly Qualitative Assurance reports on all new admits to ensure client is receiving best possible care
  • Submit weekly Qualitative Assurance reports on 4 random discharged and current clients to ensure clients are receiving consistent medical and clinical care
  • Compile and submit monthly operational report that includes statistical information derived from QA reports, as well as KPI's to ensure consistency of care for all clients that were admitted in the prior month
  • Resource and schedule all medical and clinical needs with regards to aftercare planning
  • Address all legal communication for clients via email, phone or by official documentation
  • Meet with client to discuss options and recommendations for aftercare
  • Communicate with family of client with regards to aftercare plans
  • Coordinate with clients with regards to financial matters and assist in collecting OOP balances
  • Create financial statements, collection notices and final notices and mail to guarantor responsible for outstanding balance

  • Manage client's personal cash accounts and provide access for weekly withdraws
  • Manage client's personal, securable property such as keys, wallets and purses, and keep in secure area
  • Review and submit any HR documentation for potential or existing staff members
  • Answer incoming calls to facility and direct to proper department
  • File, secure and track all medical records for discharged clients
  • Submit all medical records request received from claims department or providers
  • Handle all new admissions processes when admissions coordinator is unavailable or on vacation
  • Schedule any transportation needs for clients, ie flights, bus tickets etc
  • Record and track all KPI information in Excel spreadsheet
  • Backup on utilization review for client insurance authorizations
