Sierra Williams

• Provide individualized support and case management services to clients receiving behavioral health and substance use disorder services working toward substance use recovery and increased independence
• Assists clients with identifying and accessing community resources related to housing, sober living, healthcare, transportation, employment, education, financial assistance, benefits, and legal needs
• Coordinate and assist with medical, dental, vision, behavioral health, and other healthcare appointments and follow-up care
• Advocate for clients and assist them in communicating with healthcare providers, social-service agencies, housing programs, legal professionals, and other community partners
• Assists clients with completing applications, referrals, releases of information, benefits paperwork, and other documentation necessary to access services
• Support clients in developing independent living, communication, coping, problem-solving, self-advocacy, and recovery skills
• Assists clients with establishing and working toward individualized treatment and recovery goals
• Participate in treatment-team collaboration and communicate relevant needs, progress, barriers, and accomplishments
• Provide recovery-focused education and facilitate groups address life skills, healthy relationships, coping strategies, recovery, and community integration
• Assists clients with discharge and transition planning, including establishing connections with outpatient treatment services, sober living, healthcare, and other continuing-care resources
• Provide transportation and community-based assistance when appropriate to facilitate access to essential services and support recovery goals
• Maintain accurate, timely, and professional documentation of services provided, interventions, client participation, and progress toward treatment goals
• Maintain confidentiality, professional boundaries, ethical standards while utilizing a person-centered, trauma-informed, and recovery-oriented approach

• Provide individualized, community-based support to adults experiencing behavioral health and substance use challenges
• Utilize person-centered, strengths-based, and recovery-oriented approach to assist clients in establishing and achieving wellness and recovery goals
• Provide education, skill development, advocacy, resource coordination, and support to promote independence, stability, and successful community integration
• Coordinate access to healthcare, housing, financial assistance, transportation, and other community resources while collaborating with clients, families, treatment providers, and community partners to promote continuity of care
• Assist clients with daily living skills, symptom management, recovery planning, and transitions between levels of care.
• Maintain accurate and timely documentation of services and client progress while adhering to organizational, ethical, and confidentiality standards

Provided person-centered residential support to adults with intellectual and developmental disabilities, assisting with ADLs, individualized care plans, behavioral and emotional support, healthcare needs, community integration, rehabilitation, and independent-living skills while maintaining accurate documentation and promoting client dignity,safety, and independence
