In home health assessments performed on health plan members. The assessments are to close gaps of care, and promote optimal health outcomes for all members. I see 6 patients every week.

• Remained current on advances in nursing care by participating in educational programs and oriented and educated new staff in nursing techniques and departmental procedures.
• Administered IV fluids, medications and blood products through peripherally inserted central venous catheter to support long-term treatment for infections, cancer or heart and kidney problems.
• Educated patient, family or caretaker about post discharge care procedures to facilitate care continuum.
• Prepared chemotherapy doses and iron infusions for patient treatments as per physicians' orders.

Independently manage general and advanced pulmonary disease including ILD, COPD, Asthma, Bronchiectasis, OSA, pulmonary nodules and chronic cough.

Independently manage COPD/ OSA, pulmonary nodules and chronic cough/ dyspnea.

In collaboration with Hospitalist/ Intensitivist MD and interdisciplinary team members, I helped manage patients with chronic disease including cancer, CAD, COPD, CVA, sepsis on medical floor/ medical ICU. This included pain management, and family- patient goals of care discussions.

In Collaboration with Supervising MD, assist with managing work-flow from ED to observation or inpatient area. Participate in daily interdisciplinary rounds for medical floor. Manage CHF, CAP, Sepsis, Respiratory Failure, renal disease, chest pain and CVA.

Under supervising MD care for patients in the immediate care center, chest pain center, psychiatric stabilization unit and Observation extension of ED.

• Performed minor surgical procedures, handling suturing and wound care.
• Evaluated patient histories, complaints, and current symptoms.
• Liaised with healthcare teams to determine best course of action for patients.
