As a hospice intake RN I review hospice referrals as they are received and coordinate admissions to hospice. I work directly with hospital discharge planners, patients/families, and community care providers such as SNFs and ALFs. In this role I provide education to patients and their loved ones about hospice, discuss insurance benefits, and evaluate that goals of care align with hospice.

As the primary RN for a SNF unit I provided direct patient care, wrote comprehensive care plans, managed admissions, transfers, and discharges from the unit, and supervised nursing assistants.

As a Medicare-A/Insurance case manager I followed skilled patients through their SNF rehab stays. I completed admissions, wrote comprehensive care plans, facilitated care conferences, completed MDSs, gave trainings on documentation, and monitored documentation for regulatory compliance.
