I am currently employed with Dale Medical Center as a medical social worker serving home health and hospice patients in their homes. Conduct home visits in a timely manner according to patients' plan of care and CMS regulations. Assess for psycho social concerns and provide education on illness and coping skills and evaluate patients social, emotional and psychological needs to create treatment plans. Also assist patients in identifying community resources and assist with rehabilitation needs and/or referrals.

I worked at Southern Care as a medical social worker to hospice patients and their families. I managed a caseload of 30-35 patients. My role was emphasizing on helping patients/families adjust to illness and philosophy of hospice care. However, respecting patients wishes at all times. I provided education and assisted patients/families on Advance Directives and DNR (Do Not Resuscitate). Formulated treatment plan strategies with multidisciplinary team members to provide comprehensive and continuous care plan.

As a Social Worker, I assessed patients needs, designed a treatment plan, implemented and provided ongoing evaluation. Documented all contacts and interactions with patients and family member and coordinated care leading toward acceptance of end of life care. I also referred patients to community resources as needed to fulfill social and emotional needs. And lastly, coordinated with care team to ensure patients overall needs are met.

This was the first job as a professional social worker. I participated in admissions of long term patients as well as short term skilled care and provided for patients social and emotional care needs aligned with federal guidelines. I also created documentation in timely manner and filed in patients medical record and coordinated patient care with multidisciplinary team. I participated in facility survey's with the Alabama Board of Public Health as well as planning for patients safety needs in an imminent weather situation.
