Assess for psycho social concerns and provide education on illness and coping skills and evaluate patients social, emotional and psychological needs to create treatment plans. Also assist patients in identifying community resources and assist with rehabilitation needs and/or referrals.
I worked at Southern Care as a medical social worker to hospice patients and their families. I managed a caseload of 30-35 patients. My role was emphasizing on helping patients/families adjust to illness and philosophy of hospice care. However, respecting patients wishes at all times. I provided education and assisted patients/families on Advance Directives and DNR (Do Not Resuscitate). Formulated treatment plan strategies with multidisciplinary team members to provide comprehensive and continuous care plan.
