• Assess client eligibility for federal, state, and local entitlement programs including Medicaid, SNAP, SSI/SSDI, and housing assistance by reviewing financial, medical, and household documentation
• Conduct comprehensive intake interviews with clients to gather demographic and socioeconomic information needed to determine program eligibility and benefit levels
• Maintain accurate and up-to-date case files in compliance with agency standards, regulatory requirements, and confidentiality guidelines
• Guide clients through complex application processes, explaining eligibility criteria, required documentation, and program benefits in a clear and accessible manner
• Collaborate with social workers, case managers, and community partners to coordinate wraparound services and ensure clients received the full range of benefits available to them
• Monitor over 70 ongoing client cases to track eligibility status changes, process renewals, and ensure continued compliance with program requirements
• Identify and resolved discrepancies or gaps in client documentation to prevent delays or denials in benefit approvals
• Advocate on behalf of clients during appeals processes, preparing supporting documentation and communicating with program administrators to resolve disputes
