-Outreach to offices via tasks to alert them patient is due or due soon for their Annual Wellness and/or Physical.
-Outreach calls to patients to assist with scheduling their appointments.
-When patient has their appointment, I include HCC conditions to their appointment to alert provider to assess/review those conditions.
-Once visit completed, review chart to see if it was diagnosed/assessed.
-If not assessed, I enter not assessed in Practice assist for those member conditions.
-Processed requests for patient information efficiently, ensuring compliance with data privacy regulations.
-Verified patient identity and authorization for record requests to maintain confidentiality and security.
-Maintained accurate and organized records of all released information to support auditing processes.
-Communicated effectively with healthcare providers to clarify information needs
-Reviews PHI using EMR as well as legacy systems.
-Processes ROI requests that needs to be sent to outside facilities, by scanning the requests into EPIC. Then either faxing or mailing via postal service
-Abstracts data elements from patients medical records in EPIC system.
- Monitors and problem solves wrong information in patients records.
- Reads and interprets various components of the EMR, such as pathology, radiology, scanned reports, physical findings, and treatment modalities, and uses that information to correctly abstract patient information.
-Meets and maintains productivity for performance standards.
-Assists users with the HPF system and may included assisting physicians with completion of their deficiencies and queries.
-Assists with the distribution and management of transcribed patient records and support for the transcription units daily functions.
-Customer support included, staff, physicians, nurses, HIM Department, Administrative personnel, QA, Risk Management, outside physicians offices,
-Prioritized STAT dictations and ensure all inquiries and STAT documents are completed in a timely manner.
