• QC Review applications that come through the que to determine if applicant is eligible for services and able to move forward with case management.
• Enters Data into Salesforce.
• Contacts applicants and provide status updates, request needed documentation, and perform general case management.
• Utilize the NCORR System of Record, Salesforce, to advance files, record information, and record call logs.
• Verify household income, verify application content for eligibility, calculate award values, create award records.
• Nintex/DocuSign to execute program agreements with landlords and tenants.
• Complete an investigation to verify if the person requesting assistance is the owner of the property, or the assigned tannate on the lease.
• Participated in clinical program design, implementation, and management, monitoring, and reporting to ensure clients are not lost to medical care and highest level of quality of life is maintained.
