  • Provide quality support coordination to individuals and families dealing with severe disabilities — visit regularly to ensure health and safety needs are met. Coordinate services to people with disabilities that support them to be healthy and safe and to lead self-determined lives to the best of their ability. Consistently assess all ongoing and unmet needs of people on the Medicaid waiver. Should concerns arise, appropriate interventions are implemented immediately and followed through for an acceptable resolution. Review, maintain and budget for everyone on the case load ensuring accuracy and proper spending of DSPD funds. Ensure billings are within the individualized budget plan for each caseload member. Advocate for budget increases for individuals in emergency situations. Update and maintain currency of all demographic information for caseload. Compile and maintain caseload files for compliance with agency standards including all eligibility information. Data input into USTEP system for all correspondence involving caseload including monthly logs, contact(s) and written, verbal/phone conversations, and face-to-face contacts, update employment records. Additional input includes scheduling and writing of the annual Person-Centered Support Plan (PCSP), incorporating the principals of Self-Determination and informed choice; equality and open communication; tailored supports specifically targeting specific need(s); review and determination of natural supports; development of support strategies that are linked to the PCSP. Complete all required paperwork ensuring division standards. Facilitation and ensuring quality treatment planning and participation by all involved in the individual's life. Assist individuals in acquiring the necessary skills to conduct their own meetings and support strategies. Complete UCAN assessments as needed. Maintain and update confidential records including personal information, medical/dental treatments, medications and any other health related information required to maintain Medicaid, Medicare, and Social Security Benefits. Review of all required paperwork ensuring DSPD quality assurance standards are met. Provide community resources to individuals and families along with crisis interventions as needed. Interaction with various outside agencies including Dept of Workforce Services, Social Security, Division of Child & Family Services, Vocational Rehabilitation, Huntsman Mental Health Institute, Adult/Child Protective Services, school districts and medical/dental/mental health professionals.
