• Utilized clinical assessment, coaching, and supportive counseling using evidence-based tools to optimize member’s /families’ personal resources. Monitor effectiveness of interventions; Reinforce treatment plan through medication compliance including support for behavioral plans for compliance; advise and coach members/families; Succinctly document interventions for coordination of care and services.
• Providence Elderplace Social Work Housing Specialist: February 2008-February 2013. Responsible for identifying and coordinating specialized residential care facilities to meet the needs of behaviorally complex participants, negotiated reimbursement, and monitored quality of placements. Developed database of housing providers to track specialty care placements and vacancies and enable team to identify residential options for participants.

• Developed grant-funded family care specialist role to meet the psychosocial needs of patients and families receiving care at OHSU's Movement Disorders Program and Parkinson Center of Oregon.
• Provided counseling, care coordination, advocacy, and education to patients and families through initial diagnosis through end of life.
• Planned and facilitated classes for patients and their family caregivers on self care, resiliency, and planning for long term care support.
