• Provide in home independent living skills training and assistance to individuals with diagnoses of a Mental Illness and/or Brain Injuries

• Work towards goal plans with clients (i.e. organizational/cleaning, coping strategies, community integration/socializing, budgeting, housing/work, etc.)
• Provide transportation to assist clients with community related goals

Housing Coordinator (05/2019 - as needed through 10/2020):

• Assist clients on case load in looking for flat rent, voucher, and low income housing using their current budget
• Work with local agencies to provide resources for clients in moving process (i.e. security deposits, moving costs, and application fees)
• Assist clients in applying for low income housing programs/voucher lists

• Provide Case Management services to individuals residing in the St. Paul Higher Ground residence of Catholic Charities
• Complete yearly and quarterly assessments with clients to determine progress on life goals
