Key Responsibilities

• Provides comprehensive case coordination for children diagnosed with a range of behavioral health, developmental and medical diagnoses who are placed in therapeutic foster homes.
• Takes primary responsibility for overseeing that children’s emotional and mental health needs are being met in the TFC program.
• Responsible for documentation, filing and maintenance of child records in accordance with TCYFS policy, licensing and funding source requirements.
• Participates in problem-solving and incident management for TFC program.

• Case Managers at Daybreak, Depaul USA’s day resource center in Macon, GA, help program participants build lives of greater dignity and independence.
• The Case Manager also provides direct assistance, coordinates care, and devises support plans for each participant.
• Provided crisis intervention support for clients in emergency situations.
• Facilitated communication between clients, families, caregivers, social services and other agencies to ensure client needs were met.
• Assessed clients' needs, developed service plans and monitored progress towards goals.

• The Shelter Advocate at Crisis Line & Safe House of Central Georgia serves as the motivator, mentor, and supervisor working with clients and ensures that each client is given every opportunity to gain a life free of domestic violence.
• Provided crisis intervention and advocacy services to survivors of domestic violence, sexual assault, and stalking.
• Conducted safety planning sessions with survivors to assess risks associated with leaving an abusive partner.
• Maintained detailed records of client interactions in accordance with agency policy.
• Assisted clients in understanding their rights under the law and how to access resources available within the community.
