Responsible for assisting clinical staff in the maintenance of the client information in the health record including, but not limited to, hospitalization admission and discharge, consultation forms filled out by providers, refusal of services documentation and updating when a new provider is working with a person served.
Medical records updating and tracking annual health exams (physical, eye and dental) via exel. Requests of information to obtain records. Keeping up to date authorization of information forms for all persons served enrolled in program.
Advocation for persons served to relay medical and psychiatric concerns to appropriate providers. As well as contact emergency services or mobile evaluation crisis team per request of persons served or clinical program team.
Scheduling medical appointments per request of providers and person served. Surgery coordination such as assisting person served with over the phone intakes radiology, anesthesiology and surgical intakes.
Scheduling transportation via request of person served, medical provider, psychiatric provider, visiting nursing agencies and program nursing manager and clinical staff. Educating on the use of medical transportation provided through medication insurance to both person served and provider if needed to promote independence in the community.
General supportive listening to persons served and advocation to use resources and promote independence in the community. Redirecting persons served to the appropriate team member when needed. Updating team as needed.
Assisting office manager with daily task, answering phones, filing, and documentations as needed. Ordering interpreting services as needed. documenting use of company credit cards per protocol and tracking for purchases.

Support individuals in the completion of Activities of Daily Living tasks, which include but are not limited to cooking, cleaning, personal hygiene, shopping, and money management.
Use supportive and adaptive, health-related protections/equipment as prescribed.
Assist in the development and implementation of Individual Service Plans (ISP’s) including implementation of behavioral plans and medication treatment plans.
Follow the Department of Public Health’s (DPH) Medication Administration Program (MAP) policies (when applicable), which include, but are not limited to training requirements, medication administration, medication security, and corresponding documentation.
Utilize Positive Behavior Supports practices to create a thoughtful and supportive environment by ensuring activities, plans, and routines reflect the preferences, needs, and goals of individuals.
Learn and utilize communication tools which include but are not limited to email, tablets, and letter boards, taking into consideration the method most suited to individual needs. Communicate in the preferred method and language of those you provide support.
Assist with access to community, including transportation as needed, to individuals served to access community. Examples include but are not limited to transporting individuals served to and from day programs, work, medical appointments, to family and friends visits, and other recreation and leisure activities as assigned.
