• I evaluate and assess the discharge planning needs of all patients being admitted into the hospital.
• I speak with the patient, family, Physician, Therapy Team, and Nursing team to coordinate a smooth transition back home after Inpatient Physical, Occupational, and Speech Therapy.
• Many patients not only require therapy but require complex medical planning as well.
• I coordinate care with the insurance Case Managers and send updated clinicals daily.
• I order recommended DME and Home Health upon discharge.
• I coordinate transportation home or to SNF and set up all follow up medical appointments needed after discharge.
• I assist patient/family with completing DNRs and Advance directives.
• I complete Plan of Care/Care Plans and hold family meetings at the bedside as needed.
