The Day of appointments

  • Obtain copies of any medical information, consults sheets needed to submit at appointment
  • Notify Dining services if a lunch or supper meal is needed to be prepared for the resident while away at appointment.
  • Escort resident to transportation vehicle and assist with getting on board. Provide seasonal attire, blankets, as needed.
  • Upon return to facility, assist resident off the van.
  • Notify person in charge that you have returned.
  • Provide any paperwork from appointment to nurse/person in charge.
  • Assist resident back to room as needed.
