The main responsibility is to enter referrals for hospice patients and build the medical chart for SOC. Referral information comes through several different ways such as Focura, Epic, Plink, LexMed, email and Fax. Along with referrals a multiline phone system where we answer calls from patients, pharmacy, DME companies, nurses, doctors and hospitals. The position also included afterhours assistance with triage, taking calls from patients, assisting with financials and assigning visits to medical staff.
