Licensed Practical Nurse will perform clinical and operational processes related to transition / coordination of care/case management of Post-Acute Care services for DME. The Nurse is responsible for ensuring the member is receiving all services or equipment as ordered by the physician. May assist with the coordination between providers and members.

Completed required training by DAD’S; monitored all Governmental and accreditation regulatory (CMS, Community Health Accreditation Program) reports and regulations to maintain compliance. Direct oversight of medical charts and coding, ICD-9 and ICD-10. Attended Lisa Selman-Holman coding and chart training to assist with Medicare/Medicaid compliance within medical records. Exported all Medicare information to CMS, assisted with surveys and development of plan of care. Proficient in Microsoft Office products ( Access, Excel, and Power Point ).
