I am responsible for coordinating patient referrals, establishing relationships with local hospitals, and educating the community about hospice services. Key responsibilities include:

• Coordinating patient referrals to hospice services, and ensuring continuity of care.
• Educating the community and referral sources about hospice care benefits.
• Conducting sales calls to existing and potential referral sources.
• Maintaining a sales territory and finding new opportunities to meet monthly sales goals.

• Regularly engaging with 75 existing and potential referral sources, such as Case Managers, Admissions & Rehab Directors, Social Workers/Social Services, Home Health Companies, and other Community Marketing Liaisons.
• Planning and executing community events that are strategic and align with Benchmark's mission and goals.
• Community outreach and education on what our services are and differentiating factors.

• Manage multiple volunteer-led constituent events simultaneously, resulting in growth year over year.
• Maintain year-round relationships with key corporate sponsors and constituents to ensure involvement in all relevant chapter activities, and recognition opportunities.
• Build sustainable corporate and community relationships that contribute to achieving event revenue goals, greater community participation, and heightened brand awareness.
