The goal of the SOS Peer Specialist is to conduct community outreach to unsheltered homeless persons, apply evidenced-based interventions consistent with a Critical Time Intervention (CTI) approach, and facilitate participants' access to housing and other essential health and social welfare services. The SOS Peer Specialist will perform persistent and assertive outreach and engagement using strengths-based approaches, continually assess the health and social needs of participants, and promote participants' access to housing and other services conducive to their recovery and other desirable outcomes. The SOS Peer Specialist will also provide post-housing care coordination services to participants and collaborate with other members of the SOS: CTI Team and community stakeholders in their activities.
Being in position to assist clients suffering from substance abuse alcohol abuse mental health and
homelessness, or re-entering the community from jails, halfway houses and other inpatient facilities.
Educating clients on the multiple ways of recovery and how to live clean. Providing assistance with housing,
resumes and job referrals, and resources for the next steps to help the clients live independently. I
perform Street Outreach to targeted homeless and drug abuse areas, distributing hats and gloves, safe
sex kits, water and snacks. Developing safety protocol with the clients so when they are ready for
assistance or when situations arise, and from the established relationships they can contact our agency.
