Front Door Facilitator: Conducted front door information sessions for families and schools. The primary task is linking individuals' needs to services that are supported by OPWDD. To assist individuals, families, and advocates in learning about appropriate services that are person-centered and tailored to their personal needs. To ensure that individuals and their families are aware of the services that provide a full range of resources available within their communities. To ensure the services provided will enhance their quality of life.
Eligibility Specialist: Reviewed documentation received to determine whether or not applicants for various programs meet the necessary qualifications to apply for OPWDD-authorized services. Initiate the procedure to grant, deny, or modify eligibilities, ensuring that all eligibility and intake information is properly handled, while paying special attention to confidentiality.
Medicaid Service Coordinator: Provided case management services for individuals with developmental disabilities. Made monthly face-to-face visits with individuals at their residences, hospitals, and/or day programs. Maintained monthly notes. Maintained contact with servicing agencies, and referred individuals to Article 16 clinics. Ensure that Willowbrook Class individuals are receiving services as per New York State guidelines. Monitored individuals in IRAs, family care providers, and voluntary agencies. Attended quarterly, semi-annual, and annual meetings. Advocated for individuals when necessary, monitored, and investigated incidents as they occurred in the group homes.
