I have traveled roughly 500 miles a week in my liaison position prior to Covid. Since Covid I work remotely and go to the hospitals as needed.

* I average 15 to 20+ admissions a week

* I meet with case managers on a regular basis and work with the Director of Case management on a regular basis at all my primary accounts.

* I attend hospital meetings for long stay pts. and assist with pts. that we may be able to place amongst the Genesis centers.

* My goal is to be able to place every referral that is sent to me. To make that happen it is important to have close working relationships with case managers, Doctors, and your admissions team.

* I attend census and budget meetings, run the meeting occasionally

* I accept and deny all admissions based on my clinical judgement and experience. If I am uncertain I reach out to my colleagues. If pt. is a denial I discuss with the hospital why I am denying and what would need to be done in order for me to clinically accept pt.

* I prep all my admissions directors with what I have accepted for admissions, make sure they have wound vacs ordered, bariatric equipment, bipap/cpap orders for resp. therapists, all other important information prior to pt. admitting to center.

* Work with our Insurance team for authorizations, make sure assessments are done, give them a heads up on any carve outs we need for high cost meds.

* familiar with high cost meds and work with case managers and doctors on requesting med to be on hold or a generic form available

* Check with centers to confirm clinical capabilities, such as IV frequency's and dialysis availability.

* I work with admissions directors on Marketing events that may be going on at our centers or out in the community

* Once a month I work from my 2 primary centers for Genesis so I can talk with the team and discuss any admission barriers or about past admissions that may have not gone as smooth and how we can improve moving forward

* I also assist with the interview process for Admission Directors.

  • Worked as an Admissions Director at Oak Grove Center in Waterville. While working as an Admissions Director I was also a Clinical Liaison (Hybrid) for Genesis covering all referrals from Maine General Medical center and Northern Light Inland Hospital for all 11 Genesis centers in Maine.
  • * At Oak Grove, attend morning meeting and then go to Maine general and attend rounds with the case managers.
  • * While I was at the hospital I would talk with pts. at bedside, families, case managers, therapists, Doctors, etc.
  • * Maine General had a small office to work from to attend a meetings or help them with a particular case
  • * In the afternoon I would head back to Oak Grove and make sure rooms were ready for admissions, talk with nurses and staff to let them know about admissions, submit for insurances if I had not done it at the hospital, give tours, attend meetings, have pts. sign admissions contracts, follow up on dc calls, family calls, etc.
  • *While at Oak Grove it was expected to attend Chamber meetings, after hour Business expo's, Senior Expo's at Colby college, worked closely with Nale and Nale law, worked diligently with hospice agencies, Assistive living facilities.
  • * Built a strong working relationship with staff at Oak Grove. It is important as an Admissions Director to not only have close working relationships externally out in the community but also internally. A strong Admission Director should have an excellent working relationship with there nurses, therapists, business office manager, dietary, CNA's, Administrator, and DON.
  • * While I was doing all the Admissions work for Oak Grove I was also reviewing every referral out of Maine General and Inland Hospital for all Genesis facilities in Maine. I also facilitated referrals to out of state centers for Genesis and took referrals from out of state as well.
  • * Averaged 15 to 20 admissions a week while working as a Hybrid during this time and still managed all the Marketing expectations and Admission Director expectations.
