Behavioral Pavillion

-Responsible for the medical and psychological management of patients withdrawing from various substances.

- Assessing patients, administering medications, monitoring withdrawal symptoms and providing education and support.

- Intake screenings and ongoing assessments, monitoring patient's conditions, identifying and assessing risks, including risk of harm to self and/or others.

-Developing and implementing care plans,

-Crisis intervention.

-Served as a Treatment Plan Coordinator and developed treatment plans.

-Collaboration with other treatment team members.

- Prioritizing and organizing the daily care of patients.

-Assist with and perform exams and procedures.

-Prepare and operate necessary equipment and instruments for examinations, treatments, and procedures.

- Administration and documentation of medications.

-Maintain working knowledge of current issues and treatment modalities.

-Demonstrates an understanding of mental illness, addictions and correctional managed care.

- Led patient assessments and developed individualized care plans based on clinical evaluations.

- Collaborated with interdisciplinary teams to coordinate comprehensive patient management strategies.

- Educated patients and families on health conditions, treatment options, and preventive measures.

- Responsible for providing a full range of medical and psych services to young and senior patients in extended care facilities.

- Involved in the launching of the GRACE

(Geriatric Resources for Assessment and Care of Elders)

Program.

- Publication of GRACE research, study funded by NHI in July 2006.

- GRACE Program was shown to improve quality and outcome of care in low-income seniors and reduce hospital utilization in a high risk group.
