I am currently a Registered Nurse with a Bachelor’s degree working as a Clinical Admissions Liaison at Encompass Health Rehabilitation Hospital of Gadsden. I receive referrals from multiple referral sources via fax, phone, and online portals and I then notify the Rehab Liaisons assigned to those territories of the referrals. The rehab liaisons work up the referrals and I assist with any clinical questions they may have, review the prescreen/work up of the patients they complete for our medical director and sign them if completed by a non clinical liaison. Our medical director will then approve the patient or request additional information. I will assist the liaison in getting the needed info. I will submit to insurance for pre authorization if needed. I gather all the needed clinical information needed for the hospital and loaded it into their chart to be available for viewing by hospital staff. We also gather information such as medication reconciliation, wound care and needed supplies, verify weight bearing status, diets, and any needs the patient may need or have prior to being admitted. We also assist with insurance peer to peers and help completed MD appeals with our medical director if the patient is appropriate. We set up transport for the patients to get to our facility and speak with family members if needed. We give hospital tours and help market our inpatient rehab with out rehab liaisons. We also review diagnosis for appropriate documentation to assist our MD with accurately diagnosing patients by CMS guidelines. We review appropriateness of patients from prior level of function and current level of function to ensure their ability to participate in 3 hours of therapy per day and appropriate patients are being submitted to managed care insurances. We verify insurance benefits and coordinate their benefits with the patients, family, liaisons and referral sources.
