Patients are often forced to seek ED care for exacerbations of chronic disease. Patients presenting to the ED at RWJ University Hospital Rahway were released to the community without identification of upstream clinical or social drivers or a safe plan for discharge to access care for chronic disease. We identified:
1.Cardiology represents the highest Contribution Margin (CM) and volume of chronic disease DRG’s presenting to our ED.
2.Hypertension (HTN) a gateway diagnosis/upstream clinical driver (often undiagnosed and uncontrolled) leads to cardiovascular events, financial costs and mortality.
