As SSD I was involved in each department's growth. I have been in SSD position for more than 25 years. I also worker in a hospital setting as a Acute care manger. My duties were assisting with D/C goals, MDS, C/P related to t MDS and mood and behaviors. My involvement with a ST resident would be addressing there needs from the time they come into a facility until they leave. I chart on different areas throughout the residents stay. Psychosocial needs and ancillary needs. I'm involved with setting up meetings with resident and families. I am involved with PCP logs, Pasrr and cut letters. Audits when needed. Reviewing Advance Directives annually. Ombudsman audit monthly. Room changes. Filling out transfer sheets. Abuse issues and grievances. Applying for Guardianships. Doing assessment that are need for UDA. Other related duties when asked. Working with insurances for NHT program.
