• Coordinated care with Day Habilitation, Doctor's offices, Pharmacy, Occupational Therapist and others for each individual
• Ensuring care is provided according to OPWDD standards

My duties as a DSP included and we're not limited to:

• Gather and record various data daily by accurately documenting
• Recording data for billing and daily activities of clients
• Gathering data for the clinical teams (ie: behavioral, nursing data, wellness checks, body checks, med administration...)

• Completed regular inspections and maintenance actions, as well as basic equipment repairs, to keep equipment operating at peak levels.
• Operated with safety and skill to avoid accidents and delays.
• Demonstrated safe driving by following regulations and safety procedures, resulting in zero accidents.

My duties as a Home Health Aide were limited to the following:

• Assisted patients with dressing, grooming and feeding needs, helping to overcome and adapt to mobility restrictions.
• Traveled to clients' homes to complete healthcare services and promote continuity of care.
• Provided mobility assistance such as walking and regular exercising.
