  • I am currently working as a medical Clinical Liaison for St Mary Rehabilitation Hospital, where I travel to different hospitals. I have great skills in communication and clinical knowledge to effectively manage patient referrals. Understanding criteria, diagnostic case mix, payer requirements, and transitions of care. Assessing patient eligibility for acute rehab services by identifying and assessing potential patients, conducting pre-admission screenings, and reviewing medical records. Developing and building strong relationships with referral sources, such as physicians, case managers, and other healthcare professionals. Provide education to patients, families, and healthcare staff on acute rehabilitation services, eligibility, and processes. Work with the EPIC (Electronic Healthcare System). Excellent verbal, written communication, and time management skills to effectively communicate with patients, families, and healthcare teams.
  • I worked for APIS as an LPN, where I gained experience in drug and alcohol recovery and with mentally ill patients. I collaborated with an interdisciplinary team of healthcare and social service providers to address patients' needs through effective intervention and care planning. Performed routine evaluations of each patient's status, needs, and preferences. Assisted physicians with check-ups by obtaining vital signs, EKGs, urinalysis, urine cultures, PPDs, injections, and COVID testing. Performed intake assessments, made appointments, set up transfers and discharges, and administered Suboxone and Methadone. Maintained supplies and medication orders.
  • I worked as an LPN in LTC at the Greenwood House as a team leader and unit nurse with 20 patients, which included, but was not limited to, administering medications by mouth, injection, G-tube (also feedings), administering controlled narcotics, taking vital signs, providing oxygen, administering breathing treatments, performing EKGs, giving injections, conducting phlebotomy, performing catheterizations, providing wound care, conducting bladder ultrasounds, performing COVID testing for patients and employees, electronic charting, and documentation.
