- Complete bedside evaluation on new referrals during their hospital stay (Carroll Hospital, Northwest Hospital, Sinai Hospital, Johns Hopkins Howard County Hospital, Hanover Hospital, York Hospital are my main accounts but would travel where necessary)
- Coordinate admission to facilities from start to finish, beginning with reviewing clinical needs to ensuring facility is capable of managing care along with communicating equipment/specialty needs, client preferences, etc.
- Develop and maintain relationships with referral sources at both hospital and community levels
- Collaborate with local organizations including Chamber of Commerce, senior networking groups, senior living , etc. to strengthen partnerships, facilitate resource sharing and educate the community
- Evaluate resident feedback to identify areas needing improvement in service delivery and community involvement
- Stay abreast of competitors current programming, pricing and initiatives and work with regional team to develop new strategies and partnerships
- Attend and coordinate community marketing events, expos
- Cover PTO for Admissions Directors at facilities, conduct tours, answer phone inquiries, disperse admission paperwork, activate electronic medical records
- Promoted from one facility to two after about a year of employment which has resulted in regional coverage (this is different than most of the liaisons employed at Autumn Lake)
