Accomplishments

  • Government Grant applications and post grant implementation:
  • At Doctors on Call Applied for and received CMS’ Independence at Home Grant. I successfully operationalized the program hiring staff and implementing processes that yielded the Hospital utilization results that CMS was looking for and performance incentive dollars to the company.
  • At Somos Community Care previously Advocate Community Providers PPS; I wrote the NYS DSRIP application including governance, physician corrective action plan, and the writing up of all 9 out of the 10 project descriptions and the full implementation plan. The application received the second highest score in New York State and received an award of $700M. I operationalized all projects successfully of Somos’s DSRIP program including creating the physician and partner engagement plans, hiring and training staff and creating training materials for the physician practices. Developed referral and intervention processes as well as productivity monitoring and reporting data.
  • Practice Growth
  • Rapid Care Solutions- I am the founder of Rapid Care Solutions, a Care Transitions and house calls practice. I started the company with one employee and successfully negotiated contracts with Managed Care Organizations. I created a system of care that has effectively provided increased ROI for all those involved including the over 400 Primary Care Physicians and HMO/MCOs whose patients the company cared for. The company has grown to a mid-size company and has reduced hospital readmissions by over 12%. During my time at Rapid Care Solutions is when I began the work forming Advocate Community Providers (ACP) and eventually left to operationalize ACP.
  • Sample Medical Center, was a two physician primary care office in need of growth. I turned it to a full blown Urgent Care Center with full diagnostic capabilities including full body x-rays, ultrasound, laboratory and even an onsite, physician owned pharmacy. Treatments included everything from simple nebulizer treatments to more complicated suturing, casting and burn treatment. This process required great skill in negotiating contracts with the insurance companies, knowledge of state and federal regulations, budgeting, and precise cost/revenue analysis.

  • Non-profit work:
  • I am both a board member and active member of a New York State non-profit organization named, Fuente de Amor Fe y Esperanza, (FAFE). This is an organization which serves the poor both in low income neighborhoods of Washington Heights and Inwood, as well as areas of the Dominican Republic that are severely impoverished. The organization operates through Health Literacy programs and financial assistance here in New York and in the Dominican Republic through Medical Mission work and financial sponsorships of children and elderly, as well as providing bare essentials such as mattresses and school supplies.
