- Monitors patients for evidence of danger for self, other and report to the person in charge.

• Consult with single and families to assess needs to determine appropriate services and develop goals.
• Review case progress and determine case closure.
• Help Homeless and persons with mental health problems with programs of daily living and improve coping skills in life situations.
• Provides appropriate interventions in crisis situations.
• Collects required data and completes required documentation.
• Provides transportation when needed and appropriate and linkage to community resources.
• Help members served achieve greater independence and autonomy.
• Assist members with al needs, including Activities of Daily living.
