Serve as a Liaison who acts as a bridge between hospice care providers and patients, families, healthcare facilities, and community organizations. Play a crucial role in facilitating the referral process, ensuring patients have access to hospice services that meet their needs, and offering support throughout the end-of-life journey.

• Serve as the primary contact for physicians, hospitals, nursing homes, and other healthcare providers to refer patients for hospice care.
• Educate families, patients, and healthcare teams about hospice services and the benefits of end-of-life care.
• Assess and evaluate patients’ eligibility for hospice care based on clinical criteria.
• Coordinate the admission process for new patients into the hospice program.

Assisted seniors and their families in identifying and securing the most suitable housing options based on their individual health, financial, and personal preferences. The role involved working closely with seniors, family members, and healthcare providers to guide them through the housing placement process while ensuring they receive appropriate care, support, and resources.

Responsible for driving occupancy and promoting the services and amenities of the assisted living community to prospective residents, families, referral sources, and community organizations. This role involved developing and executing marketing strategies, building relationships with referral sources such as healthcare professionals and discharge planners, and organizing outreach efforts to increase the facility’s visibility and reputation within the community.
