  • During my time with Access, I have conducted a variety of client services, ranging from comprehensive needs assessments as well as identifying clients’ housing goals, barriers, and or support requirements. I am constantly developing and implementing individualized case plans with clear strategies for achieving long-term housing stability. I provide ongoing support through referrals within our community partners such as employment services, financial literacy programs, and mental health care. I must maintain detailed, and timely documentation of case notes, service plans, and client progress or hindrance in accordance with my agency’s protocols. I must conduct home visits to assess housing conditions, offer support, and build rapport with my clients.
