Summary of Duties

• Conduct in-depth and highly sensitive phone conversations with potential clients and their families, managing an average of 30 calls per day.
• Thoroughly assess for medical eligibility for services and providing comprehensive education on the benefits and philosophies of palliative care.
• Manage high-volume faxes and electronic referrals.

Summary of Indirect-Care Duties

• Extensive daily involvement with Epic In-Basket management including phone triage, medication refills, MyChart response, delegation of high priority calls including the ability to identify a mental health or medical crisis, and coordination of care with specialty referrals.

• In-Person and Virtual patient intake via EPIC including HPI, vitals for Internal Medicine, Primary Care with OB, Pediatrics, Dermatology, Podiatry, and Sports Medicine.
