- Please bring this Admission Ticket with you on the day of testing
- The name on this Admission Ticket must
- EXACTLY match the name of the primary picture ID that you will be bringing to the test site on the day of testing
- If the name above does not match your ID, you must call Prometric at 1-800-324-4689 before your test date
- You will not be allowed to test if the names on your ID do not match the name listed with Prometric
- You are required to bring two VALID forms of ID to the testing center on the day of testing
- Be prepared to stay at the test site all day
- It is recommended that you bring a bag lunch

- Due to COVID-19, Prometric has instituted new safety procedures for all Nurse Aide testing
- Prometric requires all candidates to bring and wear a face mask for the duration of the testing event
- Failure to bring and wear a face mask during testing will result in a No-Show result and a loss of testing fee
- You will be prohibited from testing that day
