  • As a Community Health Worker with the ACS Clinical Grants Team, I am responsible for assisting pregnant patients without medical coverage to obtain the Maternal Title V Grant to receive presumptive coverage for OB visits and coverage for dental services throughout their pregnancy.
  • Patients are able to obtain the Maternal Title V grant by completing the Financial Assistance application offered through Harris Health.
  • When I am at the clinic, I assist patients scheduled for OB Screening, OB New, and OB Return appointments to complete the Financial Assistance application, review supporting documents, and submit the documents via email to the Eligibility Department.
  • I also meet with pregnant patients who have scheduled to see me and have OB appointments on a day that I am not in clinic.
  • The scope of my job is centered on assisting these pregnant patients, and as a result, my ability to assist other patients that may need additional help with the Financial Assistance application is very limited.
