Team members assist homeless people in response to requests received and while performing outreach duties. Team members works to identify and investigate homeless situations based on requests received. Providing linkage to resources available for the homeless population. Maintaining a comprehensive up-to-date resource base for homeless services and referrals. As well as maintaining up to date Clarity (HMIS) Records for services provided to clients directly in the field. Maintaining professional relationships with offices of local government, and City and County Departments. Representing LAHSA at community meetings and advocate for clients at CES and Case Conferencing meetings
