Lead LVN at a vocational college clinic, overseeing a total of 400 students. Each student is required to have a physical, labs, immunizations, (and procedures if needed). New students arrive each week, guaranteeing my need to coordinate with the Medical, Psych, and Nursing Directors, as well as counselors, transportation specialists, and the students. Required to train nurses, and delegate task. Oversee the Medication Room, administer meds, assign medication lockers to students, destruction of expired medications, and student medication reconciliation. Other duties include, but are not limited to, case management, obtaining and processing blood and urine specimen, assisting in clinic procedures, (i.e., pap smears, STD testing, oral and genital swabs), medical supply inventory, reports and data auditing, charting and obtaining records, conflict resolution, assisting the Registered Nurse Director, and reporting to the Deputy Director as needed.

IV Specialty nurse liaison for a top-ranked, nationwide pharmacy, working in sales and marketing, and bedside care for infants through geriatric patients. Responsible for coordinating with several hospitals in Los Angeles, (i.e., USC Keck, USC Norris, Good Samaritan, Children’s Hospital of Los Angeles, Hollywood Presbyterian, and White Memorial.) Coordinated patient care with pharmacies, case management, home health agencies, hospital medical teams, patients, and their families. Required to teach acute patients and their families at bedside regarding their need for home IV therapy, (i.e., IV antibiotics, total parenteral nutrition, and G- and J-tube feeding.) Virtual and telephonic patient visits. Review of charts, labs, MARS, diagnosis, and orders. Adhere to the LVN scope of practice to insure patients safe discharge plan. Coordination of care for outpatient home IV medication in collaboration with the pharmacy team to educate and deliver medication to patients’ home.

Licensed Vocational Nurse working at a magnet, tertiary hospital, working within a triad method care team, (RN Case Manager, Social Worker, and Discharge Planner). Coordination of post-acute care for cardiac-thoracic, and vascular patients, (i.e., Skilled Nursing Facilities, Community Hospitals, Long Term Care Centers, Acute Care Units/ Facilities, and home.). Arranged transportation via ambulance or gurney van, and ordered durable medical equipment, with the assistance of patient’s health insurance. Post-acute discharge plan reviewed with the patient, family, and professional staff.

Primary Licensed Vocational Nurse at a social services organization, working for a government granted wellness program, Substance Abuse Mental Health Services Administration (SAMHSA), under the direct supervision of a Registered Nurse, and the partnership of Venice Family Clinic. Responsible for the healthcare coordination of a 250, in- and outpatient caseload. Also, responsible for collecting patient health indications, obtaining vital signs, medication management, obtaining labs, 1:1 intervention, and patient education, (i.e., providing teachings, reading material, and facilitating evidence-based groups). Responsible for connecting clients to primary medical and mental health care, (i.e., PCP and therapy referrals), as well as dental services. Utilization of case management skills; finding patient resources, building files, medical and dental insurance assistance, (especially with MediCal and MediCare), and discharge planning. Created material for small projects, hosted info sessions and luncheons, and attended hourly trainings, (i.e., webinars and facility based)

Rehabilitation and Behavioral Health residential care facility, where I coordinated client appointments with doctors and healthcare professionals, provided medication, and assisted doctors with tests and treatment. Also, used appropriate resources to assist in development and implementation of individualized care and discharge plan(s) with clients and their families.
