· Intervene with patients/families for psychosocial needs, assistance, support in coping with chronic acute terminal illness.

· Provide crisis intervention to patients/families with psychosocial needs and coordinate and facilitate the development of a discharge plan of care.

· Led mutual aid group therapy sessions in different patient units within the hospital, such as Geriatric Medical, Mood Disorder Unit, and Behavioral Treatment Unit.

· Engaged with the patients to increase motivation for recovery and provide the tools for mental health maintenance.

· Utilized the Recovery Model, Motivational Interviewing techniques, and Cognitive Behavioral Therapy skills within group sessions to better facilitate the recovery of the patients.

· Developed suicide safety plans for patients with suicidal ideation.

· Completed biopsychosocial assessments for recently admitted patients and engaged in patient case management by completing referrals to assisted living facilities, independent living facilities, residential drug programs, or detox programs; consulting with patients’ families regarding patient care; and performing discharge tasks.

· Utilized MIRACLE/Cerner online program to log documentation of patient participation and progress within group therapy sessions, biopsychosocial assessment interactions, and suicide safety plan development interactions.

· Assisted in conducting investigations into allegations of child neglect, abuse, abandonment, and special cases.

· Conducted interviews and engaged with the families involved in the allegations to better understand the dynamics within the family members.

· Assessed threats of danger and the capacities of caregivers to protect the children to determine the safety of the children with the assistance of a Child Protective Investigator.

· Provided families with services to agencies and community resources depending on family needs.

· Assisted in documentation of investigations through the Florida Safe Families Network (FSFN).

· Made daily calls to elderly or disabled clients, practicing active listening to understand the client’s needs and provide proper resources, including home delivered meals, home care resources, and assessments for Long Term Care.

· Utilized online programs including ReferNET and CIRTS to log and maintain client information.
