• Engages community to promote the importance of Advance Care Planning
• Assist patients in completing their Advance Directive and / or POLST form
• Research community programs and resources and refers patients, when needed
• Assess patient in-home needs and communicate patient needs to appropriate medical personnel
• Acts as liaison between medical office and community organization

Mission Integration

• Coordinate year-round events to help those in need: School supply drive, Thanksgiving food drive, Concern America Craft Sale (helps support third world countries) and Christmas Angel Tree Drive
• Organizes and promotes volunteer opportunities for community organizations

Advance Care Planning Group Medical Visits - Program Educator

• Group class that is offered via Zoom to educate patients on importance of Advance Care Planning
• Program Educator
• Create educational curriculum

• Provide clerical, administrative, and general office support for contractor
• Answer incoming phone calls and respond to emails in professional manner
• Submit construction bids and invoices
• Responsible of keeping track of expenses of all job sites and projects
• Prepare all documents and ledgers for tax purposes

• Recorded and reported patients’ day-to-day vital signs and symptoms
• Perform personal hygienic needs: bathing, feeding
• Administrating prescription medication
