Purpose of audit:  To verify compliance with audit requirements for HCBS - Total Number of Direct Care Staff: 294, Total files audited 45 (28 ATC, 32 Habilitation, 26 Respite, and 23 homemakers).
o Result for audit:  OLCR inspection completed and passed for each site.

 The agency was fully compliant with Certification Standards with the files and records reviewed during the audit.
• Ensure participation of the company’s employees in annual and quarterly PCSP and/or IEP meetings.
• Ensure the safety and welfare of members in compliance with the Arizona Department of Economic Security - Division of Developmental Disabilities (DES-DDD) and the Arizona Department of Health Services (DHS) Licensure. Provide all aspects of supervision through verbal and written administrative guidance to the Direct Service Provider.
• Advocate for members to ensure their best interests are foremost in all decisions and actions directly affecting their lives.
• Serve as primary contact for Qualified vendor contract management with DDD/AHCCCS, parents/guardians, employees, and other agencies to provide information and support to ensure the members' needs are adequately met.
• Maintain agency/business HCBS certification as current and in good standing. OLCR certification is renewed successfully every year before the due September 30th. Always received compliments from the OLCR team for maintaining 100% compliance.
• Took initiative in 2020 and received training from Sandata to implement an EVV system to remove paper billing. Closely monitored and liaised with Sandata and Metro Care Direct Services Providers in implementing the EVV system for Metro Care Services within six months without disturbing the level of services provided to members.

• I started my career in 2004 as direct support staff at Tungland Group Home in the Phoenix area.
• Looking at my performance within 6 months I was made manager of two group homes with a total of 7 members.
• Within one year of joining Tungland I was put in charge of 12 group homes with 45 members because of my skills of Quality Management of licensing records for members and direct services providers, caring for members, and building very good relationships with family members and guardians, etc.

• Within two years made in charge of years, 25 group homes with 75 members to monitor and review 1) group homes are providing Individuals with complex needs adequate services/support and 2) ensure that group home residents are free from abuse, neglect, and exploitation. 3) members with staff ratio is maintained all the time.
• Maintain and update complete, accurate, and current information in confidential files in accordance with DES-DDD licensure and Tungland Corporation administrative policies monitoring all medical needs of individuals and ensuring all doctors’ appointments are completed on time for 75 individuals.
• Evaluating and ensuring residents of 25 group homes are receiving the services listed in their ISP/Person-centered service plan and are effective for the individuals residing in the group home. Ensuring all relevant medical documents are placed properly in the individuals’ files.
• Ensuring that any physical interventions used on individuals adhere to their PCSPs and applicable state and federal laws and regulations.
• Regularly providing training to group home staff to ensure all employees advocate for individuals and families for their best interest in all decisions and actions affecting members' lives.
• Every year two Monitoring audits are conducted by DDD for all group homes for health and safety and on other parameters. We have always received 98% to 100% ratings on all parameters.
• Reviews and investigates critical incidents impacting the safety and welfare of members, facilitates the implementation of corrective action, and ensures external reports are made and corrective action is taken, as necessary.
