• Coordinate with external agencies to ensure client needs are met in a timely manner.

Integrated Care Management is a tool available to members who are in need of community resources, referrals, and health education.

As a Care Management Coordinator I provided support to those members in need of resources. Mery Care has a close working relationship with the Department of Developmental Disabilities. Being a CMC also means building working relationships with various providers.

Case manage clients in the Families FIRST program.

Families FIRST (Families in Recovery Succeeding Together) is a program dedicated to helping families dealing with substance abuse be successful in recovery. All families have an open DCS (Department of Child Safety) case, some of which are working towards reunification.

Working with families who have had DCS come into their lives.

As a Parent Aide I was able to facilitate visits between parents and their children currently in the care of DCS. I was also able to help educate families on parenting skills that would strengthen their relationships.

Worked with children who had various developmental and physical disabilities either on a 1:1 basis or in a group setting.
